What's the correct format for a scholarship essay heading?

I'm currently working on a few scholarship essays and want to make sure that everything is formatted correctly, including the heading. I'm not sure what the standard is for this kind of essay and don't want to mess it up.

Should I include my name, the date, or the name of the scholarship program in the heading? Or is it more like a creative title situation? Any examples or insights would help ease my stress right now!

1. The name of the scholarship: Place it at the top center of the page, in bold text.

2. Your name: Below the title of the scholarship, align your name to the left of the page.

3. Your address: Write this under your name, also aligned to the left.

4. Your email and phone number: This is an optional addition. If you decide to include it, list these below the address, aligned to the left.

5. Date: The date can be placed on the right side of the page, after your email and phone number.

6. Essay Title: This isn't always required and will depend more on the nature of your essay. If your essay is addressing a specific prompt or a question, it might not need a standalone title. But if you feel a creative title will complement your essay and doesn't duplicate the headline (name of the scholarship), you may centrally align it two spaces below the scholarship name.

Remember that your scholarship essay should be in legible font size (12 pt preferably) and font type (common ones like Times New Roman, Arial, or Calibri). The entire document should be double-spaced for legibility and ease when being reviewed.
