Their responsibilities generally include:

1. Planning and assessing: They work closely with students, parents, and teachers to develop Individualized Education Programs (IEPs) and Transition Plans. These plans outline a student's post-high school ambitions – whether it's heading to college, entering the workforce, or living independently – and detail the steps needed to achieve these goals.

2. Coordinating resources: They are adept at coordinating community resources and partnering with various service providers to support students' transition to adulthood. This includes vocational rehabilitation services, post-secondary educational programs, job coaching services, and more.
