Which Colorado colleges have guaranteed transfer options?

I've heard some Colorado colleges offer guaranteed transfer programs. Does anyone know which schools have these programs and how they work? I'm trying to plan my college transfer path carefully.

The state of Colorado has a strong Guaranteed Transfer (GT) program. In this program, certain general education core courses completed at Colorado community colleges will automatically transfer to any public Colorado four-year institution, and many private institutions as well, assuming a grade of C- or better in the course.
