What is the Common App's Official College Report for transfers?

I'm using the Common App to transfer and saw there's something called an 'Official College Report.' Does anyone know what exactly this involves and how I get it filled out for my application?

The Official College Report is a form that must be completed as part of your transfer application when using the Common Application. This form is filled out by a school official—at most colleges, this will be your college registrar or an advisor from your college's transfer office or a similar position.

The Official College Report provides the college you're applying to with a short but comprehensive overview of your academic and disciplinary record at your current institution. It includes information about your dates of enrollment, grades, standing with the institution, eligibility to return (i.e., whether you're in good academic and disciplinary standing), and confirmation of earned credits.

To get the Official College Report completed, you should start by downloading or printing the form from the Common App website. You'll need to fill out the top section with your personal and educational information. Then, you'll take this form to the relevant person at your institution—often the registrar or transfer advisor—and they will complete the rest of the form on your behalf.

Sometimes, depending on the school, there might be a fee for providing this service. The registrar or counselor should then be able to either give the completed report back to you sealed in an official envelope or send it directly to the colleges to which you are applying.
