How do I ask for a recommendation letter from a teacher?

Hi all! I'm a junior and I need to ask some of my teachers for recommendation letters soon. I'm really nervous about it and I'm not sure how to approach them. Does anyone have any tips or advice on how to ask? Thanks in advance!
