I had a 4.0 for a really long time, but it dropped to a 3.95 because I got an A- in PE (missed a few classes due to panic attacks, rip). My weighted cumulative GPA is 4.17, but my extracurriculars are below average so I'm worried that I won't get into any good colleges at this point. I've just completed the first semester of Sophomore year. Is there a chance that I can raise my GPA enough by senior year? Or am I basically screwed?
Right now as a Sophomore, you are in great shape so neither your 3.95 nor 4.17 matters one bit.
But by the time you apply to colleges, you want your 3.95 to be as close to 4.00 as possible and you want your 4.17 to be like a 4.35-4.45 because that will show that you both excelled in your APs /IBs and they counted heavily to improve your WGPA.

If you define good as T20, that definition is wrong. There are many other good colleges out there where you can get a quality education. You're not screwed if you don't get into MIT or Princeton.
If your extracurriculars are below average, even a 4.0 won't be enough. A 3.95 isn't too far off from a 4.0, so you should be fine with your GPA. You need to work more on your extracurriculars rather than worrying about an A- in PE.

Depends on what college you’re thinking of mostly, but no Harvard or Yale aren’t going to scoff in disdain because you got an A- in PE.

It’s unlikely since it’s already high enough. Most Ivy League schools would probably accept you if you’re truly able to write an excellent essay which shows what kind of student/person you are. So no need to panic!
