What do I do if I had Circumstances that Dropped my GPA?

SO basically i’m going to keep this short but informative. I am a current junior in high school and I was diagnosed with POTS a year ago after being misdiagnosed with so many other things. I was barely able to go to school so my grades obviously suffered the first semester of my sophomore year. After that I switched schools and I have managed to get straight A’s, I have perfect test scores on both the SAT and ACT and I’ve taken extreme measures to raise my GPA to a 3.6/4.0. I wanna go to a prestigious ish school but I realize that’s likely an unrealistic goal but I want to at least apply so like what do I do to increase my odds? Can I explain what happened? I’m first generation so I’m very clueless here. Please help
