I handle my Instagram account and share UI/UX design content weekly. And I should list it on my common app.
So when I list this activity, do I need to include my account username or name in the description of this activity?

You should definitely include the account name, but I'd say including the username is optional. However, if your word count allows, there's no harm in entering both!
In the description, you are supposed to include what you do when your doing the activity. For example sinnce you have an instagram account, here are a few things you could write in the description - create weekly content, manage the account, engage with audience etc. If you have enough word limit left, then you can add the username.
