I am planning to cancel my AP exam but I am not sure where to find the exam code?

On the bottom left corner of this form is asking for exam codes but I don't know where to get them and also I'm kinda hesitant to ask my guidance counselor so any help would be highly appreciated.

This form is for canceling your score after you take the exam; I would contact College Board through telephone to take the necessary steps to cancel your AP exam.
