Med related ecs even if I'm going in engineering?

If I am applying to universities for an engineering major, should I include medical-related ecs in my applications? (shadowing a doctor, president of a biology club, volunteering in a hospital).

Yes, absolutely. These show commitment to something, responsibility, and commitment to your community. These will 100% help your application and show uniqueness to your application aside from the basic sports, NHS, etc.
