I go to the gym several times a week and read every day. Does that count as an extracurricular activity?

no - that’s a hobby. unless you can turn it into something that has substance, like a blog/book club/podcast etc, it doesn’t count as an EC.
of course! however, some “untraditional” activities like babysitting, having a job, mentoring, etc DO count as ECs so if you do those you can count them

Would something like family responsibilities count?
if it’s something like taking care of siblings/acting as a step-in parent, earning income for the family, working at a family restaurant, etc. then yes
Going to the gym is a very admirable thing, but it cannot be an extracurricular activity. You need to quantify the activity. Do you compete? Have you reached a milestone or earned an achievement? Those would show that you're not just there walking on the treadmill.
